Is there any document required apart from Shako Shoumei?

Yes. The car has to be registered in your name, so you need your signature certificate or Inkan (seal) certificate. Signature certificates can normally be obtained at your home country embassy. If you have an Inkan (personal seal), you can register your Inkan at your local city office and obtain an Inkan certificate from them.
